Transition to adult life can be a difficult process for anyone, especially individuals with disabilities. Transitions should be outcome based and planning long-range in nature. Transition planning for students aged 14 through graduation assists in creating a plan that will foster a smooth progression from school to the adult system, employment opportunities, and responsibilities of adult life.
